Evidence-informed clinical review. Doxorubicin and related anthracyclines are useful in several veterinary cancer protocols, but they can cause acute rhythm changes and delayed myocardial injury. Risk is not captured by one test, and the absence of a murmur does not guarantee normal cardiac function. Monitoring should be planned before cumulative exposure becomes unclear.

The clinical question

The oncology and cardiology teams need to decide which baseline and interval assessments are justified for this dog and what finding would change treatment. That decision may incorporate prior cardiac disease, breed risk, clinical signs, earlier anthracycline exposure, other cardiotoxic factors, and realistic alternatives.

What current evidence can establish

History and examination identify symptoms and overt disease; electrocardiography assesses rhythm; echocardiography evaluates structure and function; biomarkers may add selected information. Veterinary pharmacology recognizes cumulative delayed cardiotoxicity, but test schedules and thresholds are not identical across protocols or patients.

A decision-focused pathway

Create a complete lifetime treatment record, including prior anthracycline received elsewhere. Document resting respiratory pattern, exercise tolerance, cough, fainting, rhythm, concurrent medicines, and baseline imaging or biomarkers when chosen. Agree in advance who reviews each result and whether the next infusion proceeds, changes, or requires cardiology input.

When an abnormality appears, determine whether it predates treatment, reflects transient physiology, or suggests meaningful progression. The decision may involve repeat measurement, a different modality, cardiology management, protocol modification, or another cancer treatment. Tumor-control goals and cardiac safety should be weighed transparently rather than in separate conversations.

Evidence gaps and interpretation

No screening strategy eliminates risk, and a normal baseline does not promise that injury will not develop. Measurements vary with loading conditions, stress, technique, and reader. Evidence about preventive agents or alternative formulations is context-specific and should not be converted into a universal requirement or reassurance.

Safety, monitoring, and escalation

Collapse, fainting, new breathing difficulty, rapid resting respiration, profound weakness, or a persistent new cough requires prompt assessment. Owners should not add heart medicines, supplements, or salt restriction without coordinated advice. Infusion administration and extravasation precautions remain separate safety priorities.

Define the cardiac baseline before interpreting change

Cardiac risk assessment is stronger when it records pre-existing murmur or arrhythmia, exercise tolerance, relevant breed or family history, concurrent disease, blood pressure, and the reason any echocardiographic or electrocardiographic test was selected. A baseline does not predict an individual dog's outcome; it creates a reference against which later symptoms and measurements can be interpreted.

Monitoring also needs an accurate cumulative anthracycline exposure history, including treatment received elsewhere. New cough, breathing change, weakness, collapse, rhythm abnormality, or reduced exercise tolerance deserves clinical evaluation rather than attribution to cancer or age alone. Serial studies are most comparable when technique and key measurements are consistent. Decisions to continue, modify, or stop treatment remain case-specific and balance oncologic intent with evolving cardiac findings.
